Required Documents
To ensure a smooth and efficient appointment, please bring the following documents with you:
- National Identity Card (CIN)
Required for identity verification during check-in. - Health Insurance Card
Bring your insurance details, whether CNSS, CNOPS, or a private insurance provider (RMA, Saham, Atlanta, etc.).
An insurance certificate or coverage confirmation may also be requested. - Referral Letter (if applicable)
If your general practitioner or another specialist referred you to the cardiologist, bring the referral or prescription. - Medical File
Include any relevant medical information such as:
- previous ECGs, echocardiograms, Holter tests, and stress tests
- medical reports
- hospitalization records
- List of Current Medications
Provide a complete list of all medications, including dosages and supplements you take regularly. - Recent Test Results
If you have recent examinations, bring the reports and images:
- blood tests
- ultrasound
- CT scan
- MRI
- X-rays
